Pain Is More Than Tissue Damage

Chronic pain is often interpreted through a mechanical lens exclusively, and when diagnosis fails to name the source of the issue, we are often left with pain management plans, sometimes involving pain numbing substances, and we find ourselves looping through pain cycles that interfere with our routines, plans and flow of life.

While physical structures, injuries, and inflammation are real, and while we must ethically and thoroughly diagnose them appropriately to rule out pathological damage, modern neuroscience is expanding our understanding of chronic pain into something way more complex and deeply interconnected.

Pain is not always a signal from an injured body part, but an emotional experience created by the nervous system.

It emerges from an ongoing neural communication between the brain, the body, our emotions, our memories, our environment, and our perception of safety.

This is not to say that pain is imaginary.

For people living with chronic pain, the experience is absolutely real. Pain can influence movement, sleep, relationships, confidence, and the ability to engage fully with life.

However, neuroscience offers a powerful reframe:

The body is not broken.

The body is adapting.

Pain is the nervous system’s attempt to protect us.

When we touch a hot surface, pain encourages us to withdraw before serious damage occurs. When we sprain an ankle, pain guides us toward protection and rest while healing takes place. This short-term protective response is essential for survival.

The difficulty arises when the nervous system continues producing protective responses after the original threat has passed.

The alarm remains activated.

The muscles remain guarded.

The body continues preparing for danger.

A temporary survival strategy becomes a long-term pattern.

To understand chronic pain, we need to explore the neural relationship between the brain, emotions, and the body.

The Emotional Brain and the Biology of Protection

Every moment of every day, your nervous system is quietly asking one fundamental question: “Am I safe?”

This process happens largely beneath conscious awareness.

Before we consciously decide how we feel, the brain is already gathering information from inside the body, from the environment around us, and from previous experiences.

The nervous system does not simply react to life.

It predicts, interprets, and prepares.

This is why emotions are not merely thoughts occurring in the mind. They are biological experiences involving the entire body.

Fear changes breathing.

Stress changes muscle tone.

Connection changes heart rate.

Calm influences digestion and immune function.

Our emotional states are reflected throughout our physiology.

The brain structures involved in this process include the amygdala, insula, hippocampus, anterior cingulate cortex, and prefrontal cortex. Together, these regions help the nervous system interpret sensations, assign meaning, and determine how the body should respond.

The Amygdala: The Guardian of Threat

The amygdala is often described as the brain’s alarm system.

Its purpose is to identify potential threat and prepare the body for protection.

When the amygdala detects threat, it initiates a cascade throughout the nervous system. Breathing may become faster and more shallow, muscles may increase their readiness, the heart may beat faster, attention may narrow, and digestive processes may slow as energy is redirected toward survival.

These responses are not signs that something is wrong.

They are ancient biological strategies designed to keep us alive.

The challenge is that the nervous system learns.

Through repeated experiences of injury, stress, emotional overwhelm, or prolonged uncertainty, the brain can become increasingly skilled at interpreting danger.

A movement connected with previous pain may begin to feel threatening.

A bodily sensation may create fear.

The anticipation of pain may activate protective responses before the body has even moved.

We did not consciously choose the reaction.

The nervous system is doing it for us.

Somatic Interoception: The Language of the Inner Body

One of the most helpful concepts in understanding chronic pain is interoception—our ability to somatically sense and interpret the internal world of the body.

Through interoception, we experience sensations such as heartbeat, breathing, muscle tension, temperature, fatigue, hunger, and emotional states.

The insula, a region deep within the brain, plays an important role in creating this internal map. It acts as a bridge between physical sensation and emotional experience.

A racing heartbeat may be interpreted as excitement, anxiety, danger, or anticipation depending on the context and the meaning assigned to that sensation.

The sensation itself is only part of the experience.

The nervous system’s interpretation shapes the response.

This is particularly important in chronic pain.

After repeated experiences of discomfort, the brain may begin interpreting ordinary sensations through a protective lens. A normal muscle contraction may feel threatening. A familiar ache may be a trigger. A movement that was once effortless may become associated with risk.

The body begins to anticipate.

Yet this same ability of the nervous system to play out chronic activations for what it perceives as danger, can adapt and be re-trained to move away from pain towards safety.

The brain that learns protection can also learn ease.

Through new experiences, awareness, movement, and regulation, the nervous system can gradually create new neural pathways. We begin to move away from chronic pain and tension when the body receives fresh evidence that it is safe to soften. The Brain That Predicts Pain

One of the most fascinating developments in modern neuroscience is the understanding that the brain is not just a passive receiver of information.

It's constantly predicting.

Before you move your hand, your brain predicts the sensation of movement. Before you stand up, it predicts whether your body is stable. Before you step forward, your nervous system predicts whether that movement is safe.

These predictions are shaped by experience.

Every injury, emotional experience, movement pattern, and sensation contributes to the brain’s internal map of the world and the body.

This process is incredibly useful.

It allows us to move efficiently, respond quickly, and navigate life without consciously analysing every sensation.

However, when pain has been present for a long time, these predictions can become overly protective.

If bending forward was once associated with pain, the brain may begin predicting danger every time that movement occurs.

If a particular sensation was once frightening, the nervous system may begin interpreting similar sensations as a warning.

The body responds before conscious thought has a chance to intervene.

Muscles tighten.

Breathing changes.

Movement becomes guarded.

You may feel tense, restricted, or disconnected from your body.

This is not a failure of the body.

It is a learned protective response.

The nervous system is doing what it was designed to do: prevent harm.

The challenge is that protection can become limiting when it continues after the original threat has passed.

Moving away from chronic pain involves creating new experiences that allow the nervous system to update its predictions, with evidence that some sensations are actually safe to feel again.


The Autonomic Nervous System: The Bridge Between Emotion and the Body

At the heart of these protective responses is the autonomic nervous system—the network responsible for regulating functions that occur outside conscious awareness.

It influences breathing, heart rate, digestion, blood pressure, immune activity, hormones, and muscle readiness.

The autonomic nervous system allows the body to move between states of activation and restoration.

When we encounter a challenge, the sympathetic nervous system activates.

This is the part of our biology responsible for mobilisation and action. It prepares us to respond, whether that means escaping danger, meeting a demanding situation, or focusing intensely on a task.

However, when stress becomes prolonged, the body may remain in a state of heightened readiness.

The nervous system begins behaving as though the threat is ongoing.

Over time, this may contribute to patterns such as shallow breathing, muscular tension, interrupted sleeping cycles, digestive discomfort, fatigue, and increased sensitivity to pain.

The opposite state is supported by the parasympathetic nervous system.

This is the state associated with restoration, digestion, connection, and repair.

When the body feels safe, breathing naturally deepens, muscles can soften, digestion improves, and the nervous system becomes more flexible.

A resillient nervous system is not one that is always calm, but one that can move between states. It can activate when needed and return to rest when the challenge has passed.

Chronic pain often involves a nervous system that has become stuck in protection.

The pathway back is not about forcing the body to relax.

It is about creating the conditions where relaxation feels safe and natural.


The Body Keeps the Emotional Score

Our body is an emotional organism expressing itself biologically.

Every emotional experience creates a response.

Stress can increase muscle tone.

Fear can change breathing patterns.

Grief can create heaviness in the chest.

Anger can create heat and muscular activation.

Connection can create softness and openness.

These experiences are not separate from the body.

They are the body.

When emotions become overwhelming, suppressed, or difficult to process, the nervous system may develop protective patterns.

The jaw may remain clenched.

The shoulders may stay elevated.

The abdomen may remain guarded.

The pelvic floor may hold tension.

Breathing may become restricted.

These patterns are often unconscious.

The body is not deliberately holding tension.

For some people, chronic tension is the body’s way of maintaining control in situations where life has felt unpredictable or unsafe.

The protective pattern may have been helpful at one point.

The challenge is that the body can continue carrying yesterday’s protection into today’s experience.

Somatic approaches invite a different relationship with these patterns.

Instead of asking, “How do I get rid of this tension?” we begin asking:

“What is my body trying to protect me from?”

“What information is my nervous system responding to?”

“What would help my body feel safe enough to soften?”

This shift from fighting the body to listening to the body is a profound change in the healing process.


The Lumbar-Pelvic Connection: Where Internal Experience Meets Physical Expression

The lower back, pelvis, abdomen, and pelvic floor are areas where emotional, neurological, and physical patterns often intersect.

The muscles surrounding the lumbar spine and pelvis do not operate in isolation.

They are connected through nerves, fascia, breath mechanics, movement patterns, and communication pathways with the internal organs.

One important relationship is known as the viscero-somatic connection.

This describes how signals from internal organs can influence the muscles and tissues of the body.

For example, digestive stress, pelvic discomfort, or internal irritation may contribute to increased muscular guarding around the abdomen, diaphragm, hips, and lower back.

The nervous system may increase tension in these areas as a protective response.

The relationship also works in the opposite direction.

Chronic muscular holding and tension patterns can influence the internal environment of the body. Restriction through the diaphragm, abdominal wall, or pelvic floor can affect breathing patterns, pressure relationships, and the way we experience sensations from within.

This is known as a somato-visceral response.

The body is constantly communicating.

The spine communicates with the muscles.

The organs communicate with the nervous system.

The emotional world communicates with the physical body.

Nothing exists in isolation.

This is why chronic lower back pain is often accompanied by other experiences such as digestive discomfort, pelvic tension, anxiety, fatigue, or a sense of disconnection from the body.



Fibromyalgia and the Sensitised Nervous System

Conditions such as fibromyalgia provide another important example of how the nervous system influences pain.

For many years, people with fibromyalgia were misunderstood because traditional approaches often searched for a single damaged structure responsible for widespread symptoms.

However, research now shows that fibromyalgia involves complex changes in how the nervous system processes sensation.

The nervous system becomes highly responsive.

Signals that would normally be filtered as ordinary may be amplified.

The body may experience widespread pain, fatigue, sensitivity, and difficulty regulating stress responses.

This does not mean symptoms are imagined.

It means the nervous system has entered a state of heightened protection.

Understanding this changes the therapeutic approach.

Instead of only asking, “What tissue is damaged?”

We can also ask:

“How can we help the nervous system feel safer?”

“How can we support regulation?”

“How can we nurture new experiences of comfort, movement, and connection?”

The nervous system can be re-shaped.

"I can soften without losing control."

This is neuroplasticity in action—the nervous system creating new pathways through repeated experiences of safety.

The body has not betrayed you.

It has adapted.

The tension, guarding, sensitivity, and protective responses that develop through chronic pain are often intelligent survival strategies.

They developed for a reason.

Healing begins when we stop avoiding sensations and begin creating a relationship of trust with them.

Through compassionate awareness, supportive touch, emotional expression, and somatic exploration, the nervous system can gradually learn a new way of being.

A way that allows more freedom.

More movement.

More connection.

More ease.
